PRESENT your Student ID
SIGN the official logbook upon arrival
STEP 2: Payment
Kindly proceed to CMU Alumni Association, Inc. located at the Fulbright Garden for your payment
₱500.00 - CMUAAI Fee (₱400.00) + Alumni ID Fee (₱100.00)
STEP 3: Form
FILLOUT the CMU-F-4-ARL-001 Alumni Information Sheet and attach your CMUAAI Official Receipt
Do not USE YOUR INSTITUTIONAL EMAIL UPON ANSWERING THE FORM.
STEP 4: Confirmation
The OARL Staff will clear you after reviewing your submitted form. There is no need to wait at the office; please check your clearance account afterward for updates.
